  • The listed plans are our most commonly accepted insurance plans.

  • Dr. Endo is only accepting patients with active insurance benefits with carriers that she is a participating provider. She is not accepting Self-Pay/Cash patients

  • In some cases, we may not be a participating provider for all of the carrier's different plans.

  • List is subject to change and does not guarantee we accept your plan. â€‹â€‹

 

  • If you are unsure, please call the subscriber/member telephone number listed on your insurance card. A carrier representative can assist you with your specific plan/benefit coverage questions. It is the patient's responsibility to understand their benefits.

  • If you have an HMO plan and a primary care provider, it is your responsibility to obtain any necessary referral(s) prior to your appointment.

  • Referrals may be faxed to our office at- (808) 877-4363.

  • ​​​Co-payments are due when you check in for your appointment. We accept- cash, check, or Mastercard/Visa.

DR. ENDO IS NOT A PARTICIPATING PROVIDER FOR ANY MEDICAID/MEDQUEST, KAISER OR MDX/AGILON PLANS.

THOSE PLANS REQUIRE YOU TO SEE A PARTICIPATING PROVIDER WITHIN THOSE NETWORKS.

Please double check that all forms have been completed and you include:

  • Copy of the front and back of your insurance card(s) so we can verify your coverage. We will not be able to schedule any appointments without this information.

  • Incomplete paperwork will be returned to you to complete, and will delay processing of your registration and appointment scheduling.
