Before accessing our registration forms, please read the following directions/information regarding registering to scheduling an appointment.

If you are a new patient, our office requires you to request a referral from your (PCP) Primary Care Provider. 

Many times your (PCP) can resolve your concerns without you needing to see Dr. Endo since she is a specialist.

Your PCP should fax their referral and include your recent office visit notes documenting your medical concern,

treatment and/or medications.

If you have a history of skin cancer, you may bypass the referral from your PCP and send Dr. Endo your pathology reports

and last three (3) years of dermatology visit notes.

We need to receive your referral or dermatology notes/path reports in office before

we process your registration paperwork to schedule an appointment.

You will need to download and print, or create a PDF then complete our intake registration paperwork.

​

• We continue to minimize in office contact; we only allow patients in the office on the date and time of their appointment.

• We prioritize scheduling of appointments based on condition/concerns; postponing routine general skin checks and other non-urgent visits.

• If the patient needs physical assistance or the patient is a minor; we do allow one other person to accompany the patient.

• ​It is important that you verify/confirm that we participate with your insurance prior to your appointment.

• It is the patient's responsibility to understand their coverage/benefits/co-payments/deductible.

• Please contact your insurance carrier if you have any questions.

• We ask that you bring your insurance cards to every office visit. We verify coverage at every office visit.

• ​If you have an HMO plan it is the patient's responsibility to request a referral prior authorization from your primary care physician.

• ​​Masks are optional. We respect your choice.​

• Returning forms:  We are unable to receive paperwork via email or text; it is not secure, nor HIPAA compliant. Protecting your personal data is important to us.

If you should need to change or cancel your appointment kindly call us at least 24-48 hours in advance to avoid a possible $40.00 late cancellation fee.

This also allows us to call any patients on our wait list.

Appointment reminders are not guaranteed.

Please write/save your appointment to your calendar. Missed appointments may result in a  $40.00 "No-Show" fee being billed to you.
